The oral glucose tolerance test is characterized by a scarce reproducibility, in most of cases due to a lack of standardization; this has led to the production of reccommendation for the correct execution of it. In this context particular attention must be devoted to the execution of the test for the diagnosis of gestational diabetes mellitus. Gestational diabetes is a pathology whose frequency has been constantly increasing in the latest years and which, if not treated, has adverse maternal and fetal outcomes. However the glycemic levels that triggher the adverse outcome of this condition is yet not well defined. The HAPO study (Hyperglycemia and Adverse Pregnancy Outcome) has been designed to give an answer to this question. The preliminary results of the study have shown that there is a strong and continuing relationship between risk factors and bood glucose over a broad range of maternal blood glucose levels so indicating the necessity to review the glycemic levels until now considered normal and consequently the criteria for diagnosis of gestational diabetes mellitus.
