Each laboratory that is going to introduce automatic reporting of the estimated glomerular filtration rate (eGFR), can meet a lot of agreeing and disagreeing opinions in the scientific literature. Moreover, it must be ready to respond to an increasing pressure of specialist services and to patients’ unnecessary anxiety and harm in terms of getting life insurance and receiving inappropriate treatment. Notwithstanding the simplified MDRD (Modification of Diet in Renal Disease) equation does not provide a perfect estimate of GFR, the reporting of eGFR has improved the clinical interpretation of an established test as serum creatinine. The agreement among laboratory, specialist physicians, medical practitioners on how to report eGFR and on the consequently identification and treatment of patients with initial chronic kidney disease, will be the keystone for a better efficiency of this diagnostic test.
