A prospective study of 1,038 pregnancies on the predictive value of anti-Annexin V antibodies for fetal loss.
Background. Retrospective studies have demonstrated that anti-annexin V antibodies (anti-Anx V) are linked to
miscarriage. Their predictive value is, however, unknown. We have carried out a prospective study to evaluate the
relationship between anti-Anx V antibodies and the pregnancy outcome.
Methods. A serum sample was taken from 1,038 consecutive healthy women at the beginning of pregnancy. IgG and IgM anti-Anx V antibodies were measured by a
commercial ELISA method (Orgentec Diagnostika, Mainz, Germany). The cut-off value was set at 5 units/mL for
both IgG and IgM. At the end of the follow-up, IgG and IgM anti-cardiolipin and anti-b2GPI, anti-nuclear and antithyroid
antibodies were also measured in the sera of the women with miscarriage and in an equal number of women who gave birth.
Results. 116 women out of 1,038 (11.4%) had a miscarriage by the 22nd week; 10 were lost to follow-up, 10 had an induced abortion, 6 had a pre-term delivery,
and 896 carried their pregnancy through to term. An adverse outcome of the pregnancy proved to be directly related to the number of previous miscarriages (P = .008)
and the age of the woman (P = .002).
IgG and IgM anti-Anx V were present in 25% and in 27% of the women who miscarried, and in 23% (P = .7) and in
28% (P = .51) of those who gave birth (mean antibody concentration IgG, 4.2 vs 4.4 U/ml, P = .56; IgM, 3.7 vs
3.5 U/ml, P = .27). The prevalence or level of the other antibodies tested (aCL, anti-b2GPI, ANA and anti-thyroid)
were also not useful to discriminate between the risk of miscarriage and term delivery.
